(list additional locations and EINs if more than one)
Who should get updates? Let CMS know the names and contact info for anyone who needs to be kept in the loop about the Work Opportunity Tax Credit (WOTC) if separate from yourselves.
Once we have that info, CMS will put together the Agreement and ETA Form(s) for you to review and sign.
